The publisher edits a relevant sentence on a live page to include your contextual do-follow link. Because the host page is already indexed and has crawl history, the link is usually discovered and counted quickly.
There is no practical difference. Link insertion and niche edit are two names for the same tactic: adding a contextual link into an existing published article.
